To update your contact details such as phone number or address, please submit a request in PATCHS. If you wish to update your personal information such as your Name or Date of Birth, and wish to confirm that the current Date of Birth/Name shown on your GP records is incorrect, please fill in the correct details in the form below. For the changes to be implemented, you must also provide documentation proof to the practice at reception.
